This Request for Proposal (RFP) seeks agencies that will address the following things: 

a.        the documented need for comprehensive case management in the area to be served

b.        the agency's participant recruitment and retention plan
c.        to provide supportive wrap around services through partner agencies
d.        history of positive outcomes from programs with case management components
e.        the ability for the agency to provide intense, one-on-one comprehensive case management in which participants are seen more than once a month, a minimum of twice a month
f.        to achieve positive measurable milestones within each quarter for each participant
g.        to obtain positive outcome exits for participants within a measured length of time in the program not to exceed one year
h.        to provide follow-up for ex-participants for a period of twelve months from exit date

Funding will be provided for family resource coordinators who will engage in relationship building, provide assistance for other services as needed through appropriate referrals and who develop and maintain an updated case management plan through regular reassessments.  Agencies will be required to ensure that "no client falls through the cracks' during personnel transitions and there is adequate supervisions and management of the family resource coordinators.
