  For fastest service, please follow the following guidelines:

  • You must register ALL members of your family.  Doing this online will save your case worker time and allow them to focus on your request.

  • Please provide a CLEAR shot of a current PHOTO ID such as a driver's license, State ID or other picture identification.

  • If you are requesting assistance with a specific bill, please upload a CLEAR photo that shows your name and the amount due.
